Information about CLAMET

Composition:
Clamet 2.5 mg / 500 mg: Each film coated tablet contains: Glibenclamide 2.5 mg and Metformin Hydrochloride 500 mg.
Clamet 5 mg / 500 mg: Each film coated tablet contains: Glibenclamide 5 mg and Metformin Hydrochloride 500 mg.
Dosage:
General consideration: Dosage of clamet tablets must be individualized on the basis of both effectiveness and tolerance while not exceeding the maximum recommended daily dose of 20 mg Glibenclamide / 2000 mg metformin. Clamet tablets should be given with meals and should be initiated at low dose.
Clamet tablets in patients with inadequate glycemic control on diet and exercise:
Recommended starting dose: 1.25 mg / 250 mg once or twice daily with meals.
Dosage increases should be made in increment of 1.25 mg / 250 mg per day every 2 weeks up to the minimum effective dose necessary to achieve adequate control of blood glucose.
Clamet tablets 5 mg / 500 mg should not be used as initial therapy due to an increased risk of hypoglycemia.
Clamet tablets use in patient with inadequate glycemic control on a sulfonylurea and/or metformin:
Recommended starting dose: 2.5 mg / 500 mg or 5 mg / 500 mg twice daily with meals.
For patient previously treated with combination therapy of Glibenclamide (or other sulfonylurea) plus metformin, if switched to Clamet tablets, the starting does should not exceed the daily dose of Glibenclamide (or equivalent dose of another sulfonylurea) and metformin already being taken.
Addition of thiazolidinediones to Clamet tablets therapy:
For patients not adequately controlled on Clamet tablets, thiazolidinediones can be added to clamet tablet therapy.
Overdose:
Metformin is dialyzable with a clearance of up to 170 ml/min under good hemodynamic conditions. Therefore, hemodialysis may be useful for removal of accumulated drug from patients in whom metformin overdosage is suspected.
Indications and Usage:
Diabetes Mellitus Type 2 (NIDDM, Non-Insulin Dependent Diabetes Mellitus) where diet alone or diet and sulfonylureas or biguanides cannot satisfactorily lower blood glucose levels.
